To register for any of the camps or lesson programs, you must provide all Registration Requirements below:

• 1) Fill out the registration form below, or download a PDF Registration form, fill in and and mail to address below along with Release form, Medical form and Payment.

• 2) Download a PDF of our Medical form, fill out and mail to address below.

• 3) Download a PDF of the Registration & General Release & Liability Waiver form, sign and mail to address below.

• 4) Provide payment by mailing a check to the address below:

CREDIT CARD or PAYPAL
Pay using your credit card or with your PayPal account.
Have you already registered and just want to pay via PayPal/credit card?

BY MAIL/CHECK
Make checks payable to Sportstime. Mail your check, completed registration form, medical form, and Release Form to address listed to the left.

CONFIRMATION:

You will receive an email confirmation once we have received and processed your registration forms and payment.
